Chronic Disease Self Management Program

Healthy Change, Chronic Conditions Self Management is a Free Chronic Disease Self-Management workshop.  It helps clients gain confidence in managing their disease.  They learn effective tips and strategies that will help them improve their quality of life.  It is a licensed program from Stanford University, which has been well researched with proven results.  The Healthy Change program is for adults of all ages with long-term conditions, such as diabetes, arthritis, stroke, hypertension, lung disease, cancer or fibromyalgia.  In the workshop, you will learn how to:

  • use techniques to deal with problems such as frustration, fatigue, isolation and poor sleep
  • exercise for maintaining and improving strength, flexibility and endurance
  • use of medications
  • communication skills
  • healthy eating
  • pacing activity and rest
  • how to evaluate new treatments

The program is free and participants meet once a week for a 2.5-hour class over a 6- week period.  Trained facilitators run the workshop.  Each participant receives a free copy of the “Living a Healthy Life with Chronic Conditions” Wellness Guide.  All participants must pre-register which you can do right on this website or you can call 597-8781 for more information.
