Self-Management Programs

Chronic Disease Self-Management Program
– a free six-week program workshop series aimed to help clients gain confidence in managing their disease. This program is for adults of all ages with long-term conditions, such as diabetes, arthritis, stroke, hypertension, lung disease, cancer or fibromyalgia.

Chronic Pain Self-Management Program (Healthy Change)
– a free six-week workshop series that helps people living with chronic pain to better manage their symptoms and their daily lives.

Diabetes Self-Management Program (DSMP)
– a free workshop for people with type 2 diabetes. It teaches the skills needed for day-to-day management of diabetes and related symptoms, so that individuals can “Live a Healthy Life” with diabetes and other chronic health conditions.

Healthy You
– an 8-week program designed to help you make the necessary lifestyle changes to become the healthiest version of yourself. Topics covered in this workshop series include: The Truth about Dieting; Goal Setting & Behavior Change; Mindful Eating & Emotional Eating; Carbohydrates, Protein and Fat; Meal Planning & Recipes.
